Dementia - Meeting them where they are.

My name is Sarah and when my beloved Nana was diagnosed with Dementia, we weren’t sure where to start. Below you will find some helpful steps to help you navigate this journey from obtaining a diagnosis to arranging home care, increasing support as needed, and potentially transitioning to a nursing home along with links to relevant resources that we would have found really helpful when we started this journey. 

1. Diagnosis and Early Intervention

Early diagnosis of dementia is crucial for effective management and planning. If you suspect a loved one may have dementia, consult their General Practitioner (GP). The GP can perform initial assessments and, if necessary, refer them to a specialist for further evaluation. Timely diagnosis allows individuals and families to participate in care planning and access appropriate services. HSE.ie

2. Developing a Care Plan

After diagnosis, creating a comprehensive care plan tailored to your loved one's needs is essential. This plan should address medical treatment, daily care requirements, and future considerations. Engaging with healthcare professionals and local support services can help ensure a holistic approach to care.

3. Accessing Home Care Services

Home care services enable individuals with dementia to remain in their familiar environment while receiving necessary support. In Ireland, the Health Service Executive (HSE) provides various home support services, including personal care, respite care, and assistance with daily activities. To access these services, contact your local HSE office to request an assessment of your loved one's needs. Based on this assessment, a tailored care package can be arranged.HSE.ie

4. Increasing Home Care Support

As dementia progresses, additional support may be required. Regularly review and adjust the care plan to accommodate changing needs. Communicate with healthcare providers and the HSE to request reassessments and modify support services accordingly. Additional resources, such as day care centers and respite care, can also provide relief for family caregivers.

5. Transitioning to Nursing Home Care

When home care is no longer sufficient, transitioning to a nursing home may become necessary. Selecting an appropriate facility involves considering factors such as location, services offered, staff qualifications, and the home's experience in caring for individuals with dementia. The HSE provides a list of approved nursing homes to assist in this decision-making process.

6. Financial Support: The Fair Deal Scheme

The Nursing Home Support Scheme, commonly known as the Fair Deal Scheme, assists in covering the cost of long-term nursing home care in Ireland. Under this scheme, individuals contribute a portion of their income and assets towards their care, and the HSE covers the remaining balance. The contribution is calculated based on a financial assessment, considering income and assets. Notably, the scheme includes a "three-year cap" on the contribution from the value of an individual's principal residence, meaning after three years, no further contribution from the home is required.

Complete the Application Form: Obtain and fill out the application form available on the HSE website.

  1. Care Needs Assessment: The HSE will evaluate whether long-term nursing home care is appropriate for your loved one.
  2. Financial Assessment: This determines the individual's contribution towards care costs based on their financial situation.
  3. Approval and Placement: Once approved, you can choose a nursing home from the list of HSE-approved facilities.
